It is essential that you complete the form alongside the required documents/evidence, and return both to the school office with the subject headed ‘Special Diets’.
Chartwells need these forms to put together an appropriate meal plan for your child. Depending on their allergies or intolerances, there may not be a suitable menu option available. Please note, medical evidence is required to support the request which we know can take some time to collect; as an interim solution, Chartwells will produce an “allergen friendly” menu to ensure there are further options available.
Please return your information to Chartwells at your earliest convenience to ensure they can plan a menu for your child. Please be aware, it takes around 3 weeks to create a menu for your child from the receipt of your special diets information.
